Participation in Treatment Decisions. You have the right to know your treatment options and take part in decisions about your care. You may select someone to represent you if you are unable to fully participate in treatment decisions.
Respect and Non-Discrimination. You have a right to considerate and respectful care from your health care providers without discrimination.
The Right to Privacy. You have the right to talk privately with health care providers and to have your health care information protected. You also have the right to review and copy your records and request amendments if warranted.
Complaints and Appeals. You have a right to a fair, fast, and objective review of any complaints you may have against your care givers or facility.
